About the Teamcare Program
Established in 1985, Teamcare is the largest not-for-profit Program of All-Inclusive Care for the Elderly (PACE) in the nation, that offers care to Medicare/ Medicaid dual-eligible older adults 55 and over. Our mission is to provide individualized care that empowers seniors to live safely and independently at home – and out of a nursing home. From medical care to social supports, our program offers a unique approach to healthcare that brings together all the critical resources you or your loved one may need.
A Complete Program of Care
We provide all-inclusive healthcare for our participants in their homes and at our centers including complete medical care with doctors, specialists, nurses, unlimited rehabilitation therapy, nutritionists, hospitalization coverage, pharmacy plus socialization including recreational therapy, activities, open gym, outings, meals, free transportation and so much more!
The Personalization of Our Interdisciplinary Team
As noted in our program name, we utilize a team-based approach. Every participant receives care from an Interdisciplinary Team, which includes a Doctor, Nurse, Social Worker, Physical Therapist, Occupational Therapist, Recreational Therapist, Nutritionist and Transportation Coordinator. This team meets everyday to discuss your needs as they occur. We know our participants by name and we’re able to address your needs immediately.

The CenterLight family represents a microcosm of New York with 11 PACE centers located throughout the Bronx, Kings (Brooklyn), New York (Manhattan), Queens, Richmond (Staten Island), Nassau, Suffolk and Westchester counties. And our Staff speaks approximately 25+ languages and many more dialects: Spanish, Russian, Mandarin, Cantonese, Korean, Haitian Creole, Albanian, Arabic, among many others.
